Tam Fontenot is a Licensed Mental Health Counselor in the State of Florida and is legally allowed to conduct counseling, hypnosis, hypnotherapy and psychotherapy within the scope of her training and experience. She is not a licensed psychiatrist, psychologist or medical physician and any previously diagnosed medical or psychiatric conditions must be accompanied by a referral from the appropriate physician. Tamera Fontenot is a Nationally Certified, State Licensed, Mental Health Counselor (MH5372) and Certified Clinical and Medical Hypnotherapist. She owns TampaBay Hypno-Therapy LLC and is the primary clinician and supervisor. TampaBay Hypno-Therapy LLC serves men and women of all ages, including children and teens. The practice works with individuals, couples, family units or other groups regardless of race, religion or sexual orientation.

Hours:

Office hours: 8 AM - 8 PM, Monday through Friday.
Weekend hours can be arranged.

Home Visits, Skype and Telephone Consultations are available.

Rates:

Regular rates are $100.00 per hour and apply to regular business hours and services.
Most sessions are billed at 90 minutes. Children under age 12 are billed at 60 minutes.
A non-refundable deposit of $100.00 is required to secure first appointments.
Rates may vary for other hours, services and supervised affiliates.

NOTE: If you are of limited financial means, please, do not let the cost deter you. It is the mission of TampaBay Hypno-Therapy LLC to be of service to our fellow humans, to our community and to the world. Sliding fees are available with documentation of income/expenses and your willingness to work with more limited scheduling options or interns under the supervision of Ms. Fontenot.

Group rates: Available for smoking cessation, weight loss, stress management and special interest presentations. Free or low cost groups are offered at our offices or yours.

Special Pricing Packages: Many goals are best addressed through a series of sessions, 3 being the recommended minimum for most situations. Discounts are available for the pre-purchase of session packages; 10% off ($45.00) for 3 (90 minute) sessions, 15% off ($135.00) for 6 sessions, or 20% off ($300.00) for 10 session packages. Discounts are not available for reduce fee services.

Gift Certificates: Available any time! A terrific and unique gift!

Methods of payment:

Cash, checks, Paypal and most major credit cards are accepted. Insurance is not currently accepted. You can be provided a receipt to be submitted to your insurance company for reimbursement, though responsibility for determining your benefit eligibility would be your own.
